We are seeking a compassionate and dedicated PRN Hospice Nurse Practitioner to join our team at Inspire Hospice Newnan. In this role, you will provide exceptional palliative and hospice care to patients in the Newnan, GA community, ensuring comfort and quality of life during their end-of-life journey. You will collaborate closely with a multidisciplinary team to deliver personalized care plans and support patients and their families with empathy and professionalism.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans tailored to patient needs.
- Manage and adjust treatment plans, including pain and symptom management, in collaboration with the hospice team.
- Provide education and emotional support to patients and families regarding hospice care and end-of-life options.
- Coordinate with interdisciplinary team members including physicians, social workers, and chaplains to ensure holistic care.
- Document patient care activities accurately and timely in compliance with regulatory requirements.
- Be available to provide PRN coverage, responding to clinical needs as they arise.
Requirements
- Current Nurse Practitioner license with prescriptive authority in the state of Georgia.
- Certification in Hospice and Palliative Nursing (CHPN) preferred.
- Previous experience in hospice, palliative care, or end-of-life nursing preferred.
- Strong clinical assessment and symptom management skills.
- Excellent communication and interpersonal skills, showing empathy and compassion.
- Ability to work independently and as part of a multidisciplinary team.
