Job Description

We are seeking a compassionate, team-oriented Registered Nurse to provide a full range of professional nursing services to clients of all ages, including admission, evaluation, and care plan development. As part of a dedicated team of homecare professionals and paraprofessionals, our nurses play a key role in achieving the highest level of coordinated care for clients in Caledonia and southern Essex counties.

Responsibilities

• Conduct client admissions, evaluations, and care plan development.
• Collaborate with homecare professionals to provide coordinated care.
• Administer medications and monitor patient conditions.
• Educate clients and families on care plans and health maintenance.

Required Experience / Certifications / Licensure

• Current Vermont Registered Nurse license.
• One year of prior clinical experience required.
